The LVN Case Manager, in partnership with the Medical Management team and Physician Leadership, will reputed company case management, coordination of care and medical management consulting reputed company an interdisciplinary team. Required Qualifications:
- reputed company unrestricted NY LVN license
- 2+ years of experience in a physician's office, clinical or hospital setting
- Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD

Preferred Qualifications:

The hourly range for this role is $19.86 to $38.85 per hour based on full-time employment.
